COST: $200 Until June 5th, $225 after June 5th
There is a $25.00 Registration Fee (NON REFUNDABLE) due with the Camp Health & Medical/Liability Release forms to be sent by June 5th. They must be notarized. Payment is to be made out to your LOCAL CHURCH. Be sure to tell your children’s director or Pastor that your child will be attending camp.Please make your church check out to AZ District SDMI.Please send child’s Camp Health & Medical/Liability Release form and payments to:

*Late Registrations after JUNE 16 must bring ALL FORMS and FULL PAYMENT ($225.00) to camp on day of registration.

For special dietary needs, we will provide a planned menu a couple of weeks before camp. If a child cannot eat what is provided, please supplement from home. Refrigeration and heating are available. This is a nut free camp.

Camp begins TUESDAY at 10:00am and concludes FRIDAY after dinner at 6:00pm
